You must use one of our current order forms to submit your film for processing and stereo mounting. The current order form expires 16-March-2004 (postmark date governs). You can print the order form in color or black and white. Fill it out legibly (please print), enclose your check or money order, and mail together with your unprocessed film using the mailing label included as part of the form. (Secure the mailing label to the envelope by covering completely with transparent tape. Ink jet inks tend to run, which could cause your film to be lost in the mail!)
Sales tax applies on orders to be shipped to customers within Texas. Select the appropriate order form in your choice of Adobe Acrobat format (.pdf) or word processor format (.rtf).
